What Is a TB Test and Why Is It Required for a UK Spouse Visa?

A TB test for UK visa purposes is a medical screening designed to check whether an applicant has active pulmonary tuberculosis — TB that is present and active in the lungs. Because active pulmonary TB can spread between people, the Home Office requires certain visa applicants to prove they are clear of the disease before they are granted entry clearance to the UK.

The rules around this were updated in October 2023 when the Home Office introduced Appendix Tuberculosis (TB) into the Immigration Rules, replacing the older rules A39, B39, and C39. The aim was to bring greater clarity and consistency to how the TB requirement is applied across different visa routes including the spouse and family visa route.

Who Needs a TB Test for a UK Spouse Visa Application?

Not every spouse visa applicant will need a TB certificate, but many will. Under Appendix Tuberculosis, you must provide a valid TB certificate if you are applying for entry clearance and any of the following apply:

You are coming to the UK for more than six months which covers the standard spouse visa and you have been continuously present in a listed country for six months or more, including any period within the six months before your application date.

You are applying as a fiancé(e) or proposed civil partner under Appendix FM. This is specifically called out in the rules, meaning even if your visa is initially for less than six months, the TB test requirement still applies to you.

If neither of those situations applies to you, you may not need a certificate. That said, the rules can be fact-specific, so it is always worth getting proper advice from UK Spouse Visa Solicitors before you assume you are exempt.

Fiancé(e) and Proposed Civil Partner Applicants

This is an important point that many people miss. Even though a fiancé visa is granted for only six months, applicants are still required to obtain a TB certificate if they are coming from a listed country. The rules treat this route separately and explicitly include it within the TB requirement.

Do Children Need a TB Test for a Spouse Visa?

Yes. Children accompanying a parent on a family visa from a listed country must also provide a TB certificate. At the approved clinic, a doctor will assess the child and decide whether a chest X-ray is necessary. Children under 11 years old will not usually have an X-ray; instead, the clinician completes a health questionnaire and makes a clinical judgement. A certificate is issued if the child is found to be clear of TB.

Do Pregnant Women Need a TB Test?

Pregnant women are not exempt from the TB requirement. However, there are options available to protect the unborn child. You can choose a chest X-ray with an additional shield during the second or third trimester, or you can opt for a sputum test instead. Be aware that the sputum test usually carries an extra fee and results can take up to eight weeks. If neither option is suitable, you may need to wait until after the birth before applying.

Which Countries Require a TB Test for UK Spouse Visa Applicants

The TB test requirement only applies if you are applying from one of the countries listed in TB6 of Appendix Tuberculosis. The list is extensive and includes countries across South Asia, Sub-Saharan Africa, Southeast Asia, Central Asia, and parts of South America and Eastern Europe.

Some of the most commonly affected countries for UK spouse visa applicants include Bangladesh, India, Pakistan, Nigeria, the Philippines, Sri Lanka, Ghana, and Kenya among many others.

If your country does not have an approved TB test clinic, you will need to attend one in a neighbouring country. This requires additional planning, including potentially applying for a visa for that country just to get your test done. It is worth factoring this in early.

You can check the full list of relevant countries and approved clinics through UKVI.

How Long Is a TB Certificate Valid For?

A TB test certificate is valid for six months from the date it is issued. You must submit your UK spouse visa application within this six-month window, with the certificate included in your supporting documents.

The date of application is the date you submit your online application form and pay the relevant fees. If your certificate expires before that point, it will not be accepted, and you will need to be re-tested at an approved clinic before reapplying.

This is why timing matters. If your visa application is delayed for any reason, an expired TB certificate can create real problems. Plan your clinic appointment carefully, ideally no later than about ten weeks before your intended application date.

What Happens at an Approved TB Test Clinic?

On the day of your appointment, you will need to bring valid photo identification, any previous medical records if you have had TB before, and your test fee. The clinic will not accept a payment from the Home Office this is a separate cost you pay directly to the clinic.

The standard procedure involves a chest X-ray to check for active pulmonary TB. If the X-ray result is unclear, you may also be asked to provide sputum samples on three consecutive mornings, which adds time to the process.

If your test is clear, you will receive a TB clearance certificate on the day. This certificate must come from a clinic approved by the Secretary of State and listed on the gov.uk website. The Home Office will not accept certificates from unapproved clinics.

Required Documents for Your TB Test Appointment

You will generally need to bring a valid photo ID and a photocopy, proof of payment of the clinic fee, and if relevant, parent or guardian ID for a child applicant, UNHCR registration documents if applying as a refugee, and details of any previous TB diagnosis or lung disease treatment.

What If My TB Test Comes Back Positive?

If you test positive for active TB, the clinic will not issue you a clearance certificate. Your diagnosis will be reported to both the local health authorities in your country and the relevant UK health bodies. You will be referred for antibiotic treatment, which typically lasts around six months.

Once treatment is complete, you can be re-screened at an approved clinic. If you receive a clearance certificate after treatment, you can reapply for your UK spouse visa. A previous diagnosis of TB will not automatically bar you from getting a visa, provided you have a valid clearance certificate at the time of your application.

Can the TB Test Requirement Be Waived?

Under Appendix Tuberculosis, a decision maker has discretion to waive the TB test requirement if they are satisfied that an applicant is genuinely unable to obtain a certificate and it is reasonable to do so based on the specific facts of the case.

This is a narrow provision and not something to rely on without proper legal advice. The merits of any waiver argument will depend entirely on the individual circumstances. If you believe you have grounds to request a waiver, speak to a specialist before submitting your application.

What Happens If You Don’t Submit a TB Certificate With Your Spouse Visa Application?

Failing to include a valid TB certificate when one is required will almost certainly result in a refusal. This is not a minor administrative oversight — it is a mandatory requirement under the Immigration Rules. If your certificate expires between submission and the date your application is considered, and all other requirements are met, you may be asked to provide a fresh valid certificate before entry clearance is granted.

Do You Need a TB Certificate for Further Leave to Remain as a Spouse?

Generally, no. The TB certificate requirement applies to applications for entry clearance made from outside the UK. If you are already in the UK and applying to extend your leave as a spouse or partner, you will not usually need to provide a TB certificate.
